Do you want to cancel your private health insurance? This step should be carefully considered and comes with a number of challenges, from notice periods to the loss of ageing reserves. Find out here how to proceed correctly and what alternatives there are.

The topic in brief and concise terms

Ordinary cancellation of private health insurance (PKV) is usually possible with three months’ notice to the end of the insurance year, but requires uninterrupted proof of subsequent insurance.

A special right of cancellation exists, among other things, in the event of premium increases or if statutory health insurance becomes mandatory (e.g. falling below the JAEG of EUR 73,800 in 2025).

The loss of ageing reserves and a renewed health assessment are major disadvantages when switching private health insurance providers; an internal tariff change can be an alternative.

Understanding private health insurance cancellation: the basics for your switch

Deciding to cancel a private health insurance policy is often accompanied by many questions. First, it is important to know that there are different types of termination, each tied to specific requirements and deadlines. Termination must always be submitted in writing and received by the insurer on time; sending it by registered post with return receipt is a secure method for this. The loss of ageing reserves is an important point that must be carefully considered, especially for long-standing policies. These reserves, which you have built up over many years, are intended to keep premiums stable in later life and may be partially or completely lost when switching to another private provider, especially for policies concluded before 1 January 2009. So before you cancel your private health insurance, you should carefully weigh up the consequences and check whether an internal tariff change would be a better option. The next section looks more closely at ordinary termination and its deadlines.

Ordinary termination: observe deadlines and formalities

Ordinary termination of your private health insurance is generally possible with a notice period of three months to the end of the insurance year. The insurance year often corresponds to the calendar year, meaning that notice must be received by the insurer no later than 30 September. However, it is essential to check the exact terms in your individual insurance contract, as variations are possible. Many contracts also provide for a minimum contract term of one to three years, during which ordinary termination is excluded. An important aspect is the statutory health insurance obligation in Germany under Section 193(3) of the Insurance Contract Act (VVG). Your cancellation will therefore only be effective if you can provide proof of uninterrupted subsequent insurance cover. This proof must be available to the former insurer no later than the cancellation date. Failure to comply with this requirement may render the cancellation invalid. The question now is under which circumstances a special right of cancellation applies.

Special right of termination: When you can terminate extraordinarily

In addition to ordinary notice of termination, there are situations that entitle you to extraordinary termination of your private health insurance. Such a special right to terminate exists, for example, if your insurer increases the premiums without adjusting the benefits at the same time. In this case, you can give notice within two months of receipt of the notification of change. The termination then takes effect at the time of the planned premium increase. Another important reason for extraordinary termination is the commencement of compulsory insurance in statutory health insurance (GKV). This may be the case, for example, if your income as an employee falls below the annual earnings threshold (JAEG), which for 2025 is €73,800. Notice of termination can even be given retrospectively for up to three months from the commencement of compulsory insurance, in accordance with § 205 VVG. Proof of compulsory insurance must be submitted to the private health insurance provider within two months of termination, otherwise the termination loses its validity. A move abroad outside the EU/EEA can also give rise to a special right to terminate. Below you will find out what to bear in mind when preparing a notice of termination.

The cancellation letter: A template for cancelling your private health insurance

A formally correct notice of termination is crucial to the validity of your cancellation. It should contain certain minimum details to avoid misunderstandings and delays. Although the cancellation of private health insurance does not need to be justified, clear wording is important. Here are the essential elements your letter should include:

  • Your full name and address

  • Name and address of the insurance company

  • Your policy number

  • The explicit declaration of cancellation (e.g. "I hereby cancel my private health insurance")

  • The desired cancellation date (taking notice periods into account)

  • In the case of special cancellation: the reason (e.g. premium increase, commencement of insurance obligation)

  • Request for written confirmation of the cancellation, stating the termination date

  • Place, date and your handwritten signature

Make sure to use the word "cancellation" explicitly, as a reference to a mere change is not sufficient, as a ruling by the Dortmund Regional Court (case no. 2 O 209/11) makes clear. Many portals offer a "private health insurance cancellation template" for download, which can serve as guidance. Remember to choose registered post for sending. But what happens after cancellation, especially with regard to the ageing reserves?

Consequences of cancellation: ageing reserves and health assessment

Cancelling private health insurance, especially for the purpose of switching to another private provider, often results in the loss of, or only partial transfer of, the accumulated ageing provisions. These provisions are an important building block for stable costs of private health insurance in later life. For policies taken out before 1 January 2009, transferring the provisions is generally not possible at all. For newer policies, part of the provisions (the so-called transfer value) can be taken to the new provider. Nevertheless, this often leads to higher initial premiums with the new insurer, as not all of the provisions are transferred. In addition, a new health assessment is required when switching provider. Pre-existing conditions or a higher entry age can lead to risk surcharges, exclusions from cover, or even rejection of the application. These aspects should definitely be taken into account when deciding to cancel. One alternative may be the deferred coverage insurance, which is explained in the next section.

The Anwartschaftsversicherung: An option for a temporary switch

If you only leave your private health insurance temporarily, for example because of a stay abroad or a time-limited obligation to take out statutory health insurance (GKV), a deferred cover insurance policy can be worthwhile. It secures your right to return to your old tariff later without a further health assessment and taking into account your original age at entry. Two forms are distinguished:

  1. The small deferred cover: This merely secures your right to return without a further health assessment. No further ageing reserves are built up during the deferred cover period.

  2. The large deferred cover: Here you pay higher contributions, but ageing reserves continue to be built up, and your original entry age remains in place for the premium calculation. This results in lower contributions when full insurance cover is resumed.

Especially for trainee civil servants or people entitled to medical care cover, who later have to or want to switch to private health insurance, arranging deferred cover at an early stage is often a wise decision. This secures favourable terms for the future. But what if the insurer wants to cancel your policy?

Cancellation by the insurer: When is this possible?

Cancellation by the insurer: When is this possible?

Termination of comprehensive health insurance by the insurer is heavily restricted, as health insurance is mandatory in Germany. An ordinary termination by the provider is therefore excluded in the case of comprehensive medical expense insurance. Even if contributions are not paid, the insurer may not simply cancel the policy, but instead places the policyholder in the emergency tariff. However, there are exceptional situations in which the insurer may terminate extraordinarily or withdraw from the contract. This is the case, for example, in the event of fraudulent misrepresentation during the application process, such as when health questions were deliberately answered incorrectly (breach of the pre-contractual duty of disclosure pursuant to Section 19 VVG). The insurer can then contest the contract or withdraw from it within certain time limits (up to ten years in the case of intent). A serious breach of contract, such as billing fraud, can also justify extraordinary termination by the insurer. For certain supplementary insurance policies calculated without ageing reserves (e.g. some travel health insurance policies), the insurer may, in some circumstances, have a right of termination, but often only in the first three policy years. It is therefore advisable to be thoroughly familiar with your own policy terms. The next section summarises the most important expert tips.

Expert tips: How to act wisely when cancelling private health insurance

Cancelling your private health insurance is a step with far-reaching consequences. To avoid pitfalls, you should heed a few expert tips. Our expert tip: Before cancelling, always check the possibility of an internal tariff change under Section 204 VVG. Often, your own insurer offers cheaper tariffs with comparable benefits, without you losing your ageing reserves or having to undergo a new health check. Seek independent advice on this. Another important point is the seamless continuation of insurance cover; your cancellation only takes effect once proof of follow-up insurance has been provided. If you are planning a switch to statutory health insurance (GKV), make sure you are fully informed about the requirements, especially if you are over 55, as a return is often only possible in exceptional cases. If you are unsure or the situation is complex, it may be worthwhile seeking advice from a specialist insurance adviser or a solicitor specialising in insurance law. As part of our services, we offer you initial guidance and help you make the most of the benefits as a privately insured person or to arrange a change in a well-considered way.

FAQ

Do I need a template for cancelling my private health insurance?

A template can be very helpful in ensuring that all the necessary details are included and that the cancellation is formally correct. Important are your details, insurance number, the cancellation date and your signature. Many sources offer a "private health insurance cancellation template".

What notice period applies to a premium increase in private health insurance (PKV)?

If your private health insurance increases the premiums without adjusting the benefits, you have a special right to cancel. You can then cancel within two months of receiving the notice of the increase. The cancellation takes effect at the time the premium increase becomes effective.

What does compulsory insurance mean for my private health insurance cancellation?

If you become subject to compulsory insurance in the statutory health insurance scheme (GKV) (e.g. by falling below the annual earnings limit), you can terminate your private health insurance (PKV) with extraordinary notice. Under Section 205 of the VVG, this is even possible retroactively for up to three months. You must prove that compulsory insurance has begun within two months.

Do I lose money if I cancel my private health insurance?

Yes, especially if you switch to another private provider, you may lose some or all of your accumulated ageing reserves. This can lead to higher premiums with the new insurer. An internal tariff change is often the better alternative.

Can I still switch from private health insurance to statutory health insurance at over 55?

A switch from private health insurance (PKV) to statutory health insurance (GKV) is only very difficult for people over 55 and is possible only under strict conditions. In most cases, proof is required that there was compulsory insurance in the last five years or that the conditions for family insurance are met.

What is a deferred insurance policy?

An entitlement insurance secures your right to return to your previous private health insurance tariff at a later date without a new health check and often while retaining your original entry age. This is useful in the event of temporary interruptions to private health insurance membership.
